7.01.143 Responsive Neurostimulation for the Treatment of Refractory Partial Epilepsy

چکیده

[TOP] Responsive neurostimulation (RNS) may be considered medically necessary for patients with partial epilepsy who meet ALL of the following criteria:  Are 18 years or older.  Have a diagnosis of partial-onset seizures with 1 or 2 well-localized seizure foci identified.  Have an average of 3 or more disabling seizures (e.g., motor partial seizures, complex partial seizures, or secondary generalized seizures) per month over the prior 3 months;  Are refractory to medical therapy (have failed 2 or more appropriate antiepileptic medications at therapeutic doses).  Are not candidates for focal resective epilepsy surgery (e.g., have an epileptic focus near eloquent cerebral cortex; have bilateral temporal epilepsy).  Do not have contraindications for RNS placement (see Policy Guidelines).

Neurostimulation as a Putative Method for Treatment of Drug-Resistant Epilepsy in Patient and Animal Models of Epilepsy

A patient with epilepsy was shown to have neurobiological, psychological, cognitive, and social issues as a result of recurring seizures, which is regarded to be a chronic brain disease. However, despite numerous drug treatments, approximately, 30–40% of all patients are resistant to antiepileptic drugs. Therefore, newer therapeutic modalities are introduced into clinical practice in which invol...

Differential neuropsychological outcomes following targeted responsive neurostimulation for partial-onset epilepsy.

OBJECTIVE Responsive neurostimulation decreases the frequency of disabling seizures when used as an adjunctive therapy in patients with medically refractory partial-onset seizures. The effect of long-term responsive neurostimulation on neuropsychological performance has not yet been established. METHODS Neuropsychological data were collected from subjects participating in the open-label arm o...

Effect of neurostimulation on cognition and mood in refractory epilepsy

Epilepsy is a common, debilitating neurological disorder characterized by recurrent seizures. Mood disorders and cognitive deficits are common comorbidities in epilepsy that, like seizures, profoundly influence quality of life and can be difficult to treat. For patients with refractory epilepsy who are not candidates for resection, neurostimulation, the electrical modulation of epileptogenic br...

Clinical utility of implantable neurostimulation devices as adjunctive treatment of uncontrolled seizures

About one third of patients with epilepsy are refractory to medical treatment. For these patients, alternative treatment options include implantable neurostimulation devices such as vagus nerve stimulation (VNS), deep brain stimulation (DBS), and responsive neurostimulation systems (RNS). We conducted a systematic literature review to assess the available evidence on the clinical efficacy of th...

Quality of life and mood in patients with medically intractable epilepsy treated with targeted responsive neurostimulation

PURPOSE The primary efficacy and safety measures from a trial of responsive neurostimulation for focal epilepsy were previously published. In this report, the findings from the same study are presented for quality of life, which was a supportive analysis, and for mood, which was assessed as a secondary safety endpoint. METHODS The study was a multicenter randomized controlled double-blinded t...
